I have received and understand this employee incident report and any related action plan. I fully understand all expectations, both listed above and all standard expectations of EasyITGuys employment. I have been informed of any changes to my employment, and understand failure to perform as agreed may lead to disciplinary action, up to and including termination. I also understand this is not an admission of guilt, but failure to legally affirm this message with the my manager will result in me being sent home before consideration of continued employment.
